Medication refills: Request refills by phone. Call your pharmacy and have them call us (314-569-0510) during business hours to request a refill on your prescription.

Request refills by email: Do not submit a request for a refill that you need the same day, we cannot guarantee that it will be filled that day.

Vial refills (shots): Please make sure you are not due for an office visit with your new vial. If you are due for a visit, please contact our office to schedule an appointment. Also if you require a referral, we can only make new vials if your referral is current.

Request new vials by fax: Fax a copy of your completed vial sheet to 314-569-1085. Make sure you are not due for an office visit with your new vial. If you are due for a visit, please contact our office to schedule an appointment. Also if you require a referral, we can only make new vials if your referral is current.

Request new vials by e-mail: Scan copies of your current vial sheets and save them in Microsoft Word (.doc) or Adobe PDF format. E-mail them to refills.

Scheduling Appointments

By phone: Call 314-569-0510 to schedule an appointment for either one of our offices or use the on-line Appointment Request form.

By email: You can only use this service if there is a range of time that works for you and you wish to schedule a week in advance. We cannot guarantee an appointment at a certain time.

Send questions about your bill to Billing.
Send questions about your insurance to office manager.
E-mail record transfer forms to office manager. Please fax all record requests to 314-569-1085 or scan them in word format and email them to office manager.

Change of address or phone number: Download and complete the Patient Information form. Mail, email or fax to us so we can update your information. Please note we do not keep your old form. Make sure to fill out the form completely and email to front desk. For all other questions, send an email to front desk.
